Health record

How Health Memory is built

Where records come from, how documents connect to observations and why the history of changes matters more than a single result.

At a glance

  1. 01

Health Memory is connected context, not a folder of files: records retain their date, source and type of fact.

  2. 02

A document, personal observation, protocol response and doctor's review have different statuses and must not be conflated.

  3. 03

New information adds to the history; disputed or corrected records must not silently rewrite the past.

Where records come from

Sources may include uploaded documents, structured measurements, your observations, protocol responses and professional reviews. Origin and timing matter for each type. Your description of how you feel does not become a laboratory finding, and an automatically recognised line does not become a doctor's assessment.

BIOS shows only records available to the current account and permitted in that view. This is particularly important for family relationships and doctor access.

What happens after you upload a document

The original file is retained as the source, and individual facts are then extracted from it. You should be able to compare a structured record with the document. If recognition gets a value, unit or date wrong, the record needs correction while the original remains unchanged.

The same measurement name does not guarantee direct comparability. Methods, sample types, units and reference ranges may differ. Health Memory therefore retains the measurement context, not just the number.

Why a history of changes matters

One result describes a moment. A sequence can reveal stability, fluctuations or an association with an event: an illness, treatment change, physical activity or a new test. Temporal proximity does not prove causation — it only helps formulate a question.

A correction must not erase the fact that an earlier version existed. The history supports trust, auditing and an accurate explanation to a doctor.

How to keep Health Memory organised

Review new records periodically, merge only items that genuinely refer to the same thing, and mark symptoms or events that have ended. Do not delete an important source just because a newer test is available. Consult a doctor for medical conclusions, and use BIOS AI to search, build a timeline and identify gaps.
